Enrolling Your Child 

Welcome to San Ildefonso Day School! To ensure a smooth enrollment process for your child, it's important that all necessary administrative documents are submitted by a parent/guardian. Before you get started, be sure to thoroughly review the registration forms for a complete list of the enrollment requirements. We also require that a parent/guardian accompany the student during the enrollment process. Thank you for choosing San Ildefonso Day School!

Kindergarten or New Student: 

Kindergarten students must be 5 years of age by September 1 of the current school year. All new students will need to complete the registration packet and provide the following documentation.

  • Completed Registration Packet. All forms in the packet must be completed in full.
  • Birth Certificate 
  • Social Security Number
  • Certificate of Indian Blood (CIB) or Tribal Enrollment Letter. Students applying for admission must demonstrate membership in a federally recognized tribe, or at least one-fourth total degree Indian blood direct descendant of an enrolled member of a federally recognized tribe and provide a Certificate of Indian Blood (CIB) 
  • Immunization Record
  • Physical Form
  • Emergency Contact Information

Returning Students: 

All returning students will need to complete the registration packet and provide the following documentation.

  • Re-Registration Form. All forms in the packet must be completed in full.
  • Updated Immunization Records
  • Updated Emergency Contact Information

Enrollment Frequently Asked Questions 

What documents do I need bring to enroll my child? 

In addition to the items listed above, the following documents may need to be furnished: 

  1. Students that reside outside of the attendance boundaries must have an out-of-boundary waiver that has been approved by the school board.
  2. If you are enrolling a child and you are not the biological parent, you will need to provide proof of guardianship by presenting one of the following:   
    a. Legal court documents  
    b. Care Givers Affidavit   
    c. Power of Attorney 

Is any other information helpful to the process? 

Upon admission, any student with a known medical issue, including a food allergy, must disclose the information to the Principal and registrar. School officials will ensure that this information is shared with appropriate staff within the school in accordance with the school’s policy and make the necessary referrals to the school’s Section 504 Coordinator.
